PHI Scrub is ON by default
Patient identifiers are stripped from your note client-side before any data leaves your browser. Clinical notes are never stored on our servers. Your original note is never modified.
ClaimLens™ is operated by Prescott Labs. ClaimLens is an AI-powered clinical coding assistant for family medicine providers and practice managers. Questions about this policy may be directed to support@claimlens.app.
ClaimLens stores the following data:
Your account: Email address, subscription status, and preferences (default payer, visit type).
Extraction history: The billing codes we extracted (ICD-10, CPT, HCPCS, E/M, quality codes) and metadata about each extraction (payer, visit type, estimated revenue, date). This lets you review your coding history across devices.
Usage signals: Which codes you selected or deselected, and any codes you flagged as incorrect. This data is anonymous and used to improve accuracy.
Aggregate metadata: Anonymized patterns across all extractions (average conditions per visit, most common E/M levels, specialty distribution) to improve clinical accuracy rules.
Billing information: Processed and stored by Stripe. We never see or store your payment details.
ClaimLens never stores:
Your clinical notes: Note text is processed in memory and immediately discarded. It is never written to our database or logs.
Patient information: Our PHI scrubber removes patient names, dates of birth, MRN numbers, phone numbers, and addresses before any processing occurs.
Audio recordings: If using voice input, audio is transcribed and immediately discarded. Only the text transcript is processed, and it goes through PHI scrubbing before reaching our AI.
We do not sell, rent, or share your personal data with third parties for marketing purposes.
ClaimLens uses the following third-party services to operate:
AI processing of clinical notes to extract billing codes. Notes are transmitted per request and not retained by ClaimLens. Anthropic processes notes under a zero data retention policy for API calls — note content is not used for model training and is not retained by Anthropic after processing.
Authentication and database hosting for account data and extraction history.
Payment processing for Pro subscriptions. ClaimLens never sees your card details.
Web hosting and deployment infrastructure.
Transactional email delivery (welcome emails, billing notifications).
⚠ Important
ClaimLens is not a HIPAA covered entity and does not offer a Business Associate Agreement (BAA). While our architecture is designed to minimize PHI exposure — notes are never stored, PHI is scrubbed before processing — we cannot guarantee HIPAA compliance for your specific use case.
We recommend: Do not paste notes containing unredacted PHI. Use the PHI scrubber toggle (enabled by default) at all times.

You are responsible for ensuring your use of ClaimLens complies with applicable HIPAA obligations at your organization.
To improve our coding accuracy, ClaimLens collects anonymous signals about how you interact with extracted codes:
These signals never contain patient information or note content. They tell us only that "code X was deselected in the context of a Medicare office visit" — never who the patient was or what the note said.
This data is how ClaimLens gets smarter over time. Every thumbs-down you give directly contributes to better accuracy for all users.
We will respond to all data requests within 30 days.
The ClaimLens Chrome extension stores your authentication token and user preferences (default payer type, patient type, provider name, PHI scrubbing preference) locally in your browser using chrome.storage.sync. This data is not transmitted to external servers except as needed to authenticate with claimlens.app. The extension reads tab content only when you explicitly click "Pull from EHR" — it does not passively monitor browsing activity.
We may update this policy as ClaimLens evolves. Material changes will be communicated via email to registered users. Continued use of ClaimLens after changes constitutes acceptance of the updated policy.
