No. ClaimLens is a fully managed service. Sign in at claimlens.app with your email and start extracting codes immediately — no API key, no credit card required for the free tier.
Free accounts include 12 extractions per month. For unlimited extractions plus Pro features (Note Rewriter, Superbill PDF, extraction history), upgrade to ClaimLens Pro for $29/month.
ClaimLens accepts any plain-text SOAP note, H&P, or clinical encounter note. Paste directly from your EHR, dictation software, or any text source. There is no required format.
ClaimLens is designed with PHI protection as a core feature. PHI Scrub is enabled by default, stripping patient name, MRN, DOB, phone number, and address before your note is sent to the AI. Clinical notes are never stored on our servers. For formal BAA requirements, contact us at support@claimlens.app.
PHI Scrub runs client-side before your note leaves your browser. It uses pattern matching to detect and redact: patient names, medical record numbers (MRN), dates of birth, phone numbers, email addresses, and physical addresses. Your original note is never modified — only the scrubbed version is sent to the AI.
Clinical notes are never stored. On Pro accounts, the extracted codes and billing output are saved to your extraction history, but the original note text is not retained.
ClaimLens is not a covered entity and does not offer a BAA. The PHI scrubber removes patient identifiers before processing, but you should not paste notes with unredacted PHI. See our Privacy Policy for details.
ClaimLens extracts E/M codes, CPT procedure codes, ICD-10-CM diagnosis codes, HCPCS Level II codes, Medicare G-codes (AWV, BHI, tobacco, depression screening), Chronic Care Management codes (99490, 99439, 99487, 99489, 99491), Transitional Care Management codes (99495, 99496), Category II quality/MIPS codes, and preventive service codes — filtered by your selected payer.
Select Medicare, Medicaid, Commercial, or Self-Pay before extracting. ClaimLens automatically applies payer-specific rules: Medicare-only G-codes (G2211, AWV, etc.) are blocked for non-Medicare patients, Medicaid-specific rules apply for Medicaid payers, and so on.
ClaimLens applies 26 automated validation filters to your extracted codes, including: HTN+CKD combination code enforcement (I12.9), double E/M prevention, payer-specific G-code enforcement, NCCI bundling enforcement, universal ICD-10 deduplication, tobacco code payer routing (99406/99407 vs G0436/G0437), medication-diagnosis gap detection, and more. These are patent-pending.
Billable codes have clear documentation support in your note. Conditional codes need additional documentation or clinical judgment before billing — they're flagged with a warning explaining what's needed. Always review conditional codes before submitting.
ClaimLens captures all clinically supportable codes including quality/MIPS codes, Medicare G-codes, and chronic condition Z-codes that are commonly missed. Review each suggestion — you decide what to bill.
Use the thumbs down button on any code card to flag it. Your feedback directly improves future accuracy. Always verify codes against your documentation before billing.
ClaimLens is a clinical decision support aid. All extracted codes should be reviewed and verified by a qualified clinician or coder before submission. ClaimLens does not guarantee payment and is not a substitute for certified coding review.
Yes — select CCM as your visit type before extracting. ClaimLens will surface the appropriate time-based CCM codes (99490, 99439, 99487, 99489, 99491) based on your documentation, suppress standard E/M codes, and flag required CCM documentation elements including patient consent, care plan, and time documentation. CCM is primarily a Medicare benefit billed once per calendar month per patient.
Yes — select TCM as your visit type for post-discharge follow-up visits. ClaimLens extracts 99495 (moderate complexity, face-to-face within 14 days) or 99496 (high complexity, face-to-face within 7 days) and flags required documentation: discharge date and facility type, interactive contact within 2 business days, face-to-face visit date, medication reconciliation, and complexity level. TCM is primarily a Medicare/Medicare Advantage benefit. TCM and CCM cannot be billed in the same calendar month.
ClaimLens works with any EHR. Use the built-in copy buttons to copy codes formatted for Cerner, Epic, Athena, or eCW. The Chrome extension lets you code directly alongside your EHR in the browser.
Yes — Pro and Practice subscribers can generate API keys to integrate ClaimLens directly into billing workflows. See the API tab in your dashboard.
Yes — our Practice plan supports up to 5 providers for $99/month. The practice owner gets a shared dashboard, analytics across all providers, and can invite team members by email.
Yes — annual billing is available at $249/year for Pro (saves $99 vs monthly) and $990/year for Practice.
Install the ClaimLens extension from the Chrome Web Store, then sign in to your claimlens.app account. The extension opens as a side panel on any EHR tab (Epic, Cerner, Athena, etc.) — paste your note and extract codes without leaving your EHR.
Open claimlens.app in a Chrome tab and sign in. Then click the ClaimLens extension icon to reopen it — it will detect your session automatically. If the issue persists, try signing out and back in at claimlens.app.
In your ClaimLens dashboard, click 'Upgrade to Pro' or visit the pricing section. You'll be taken to a secure Stripe checkout. Pro activates immediately after payment.
Click 'Manage Billing' in the dashboard header, then 'Cancel subscription' in the Stripe portal. You'll retain access until the end of your billing period. No partial month refunds.
This is usually a webhook delay. Wait 2-3 minutes and refresh your dashboard. If the issue persists after 10 minutes, email support@claimlens.app with your account email and we'll resolve it promptly.
