By accessing or using ClaimLens™ (the "Service"), you agree to be bound by these Terms of Service. If you do not agree to these terms, do not use the Service. These terms apply to all users, including free and Pro account holders and Chrome extension users.
ClaimLens is an AI-powered clinical coding assistant designed for family medicine providers and practice managers. The Service extracts CPT, ICD-10, E/M, HCPCS, and quality/MIPS billing codes from clinical notes using the Anthropic Claude AI model, applies patent-pending post-processing validation filters, and provides revenue estimates based on RVU calculations.
ClaimLens is operated by Prescott Labs.
You agree to use ClaimLens only for lawful purposes and in accordance with these Terms. You agree not to:

ClaimLens is a clinical decision support tool. It is not a substitute for certified medical coding review, and output must be reviewed by a qualified clinician or coder before submission.
ClaimLens extracts billing codes based on clinical note content and AI interpretation. Code suggestions may be incomplete, incorrect, or inapplicable to your specific payer contract or clinical scenario. Prescott Labs makes no warranties regarding the accuracy, completeness, or fitness of extracted codes for any particular billing purpose.
All code suggestions should be reviewed and verified by a licensed clinician or certified professional coder (CPC, CCS, or equivalent) before submission to any payer. ClaimLens does not guarantee reimbursement, and Prescott Labs is not liable for claim denials, audits, or compliance violations arising from use of the Service.
AI-generated codes may contain errors including codes that do not exist or are clinically inapplicable. Always verify all suggested codes against current CMS code sets before submission.
ClaimLens™ is a registered trademark (Serial #99689054, Class 042, filed March 7, 2026) of Prescott Labs. The post-processing validation system is covered by a provisional patent: "System and Method for AI-Assisted Clinical Coding with Post-Processing Validation."
All software, prompts, algorithms, and content comprising the ClaimLens Service are the exclusive property of Prescott Labs. No license is granted to reproduce, modify, or create derivative works from any part of the Service.
You retain ownership of clinical notes you submit. By using the Service, you grant Prescott Labs a limited, non-exclusive license to process note content solely for the purpose of providing code extraction output. Clinical notes are never stored or used for model training.
Pro subscriptions are available as monthly ($29/month) or annual ($249/year) plans via Stripe. Both plans auto-renew. Billing begins immediately upon upgrade. You may cancel at any time through the Stripe customer portal accessible from your dashboard — Pro access continues through the end of the current billing period.
Annual plans are non-refundable after 30 days from purchase. No refunds are issued for partial months on monthly plans. Prescott Labs reserves the right to change pricing with 30 days advance notice to active subscribers.
You may terminate your account at any time by contacting support@claimlens.app. Prescott Labs may suspend or terminate accounts that violate these Terms, engage in fraudulent billing activity, or abuse the Service. Upon termination, your right to use the Service ceases immediately.
To the maximum extent permitted by law, Prescott Labs, its officers, employees, and agents shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of ClaimLens, including but not limited to: claim denials, audit findings, compliance penalties, or lost revenue.
Prescott Labs' total liability for any claim arising from use of the Service shall not exceed the amount paid by you for the Service in the three months preceding the claim.
ClaimLens relies on third-party AI infrastructure (Anthropic Claude API) to provide extraction services. Prescott Labs is not liable for service interruptions, degraded performance, or output changes caused by third-party API unavailability, maintenance, or model updates.
In the event of extended API unavailability, Prescott Labs will make reasonable efforts to communicate impact to subscribers.
These Terms are governed by the laws of the State of Nebraska, without regard to conflict of law principles. Any disputes arising under these Terms shall be resolved in the state or federal courts located in Nebraska, and you consent to personal jurisdiction in those courts.
ClaimLens collects anonymous usage signals to improve coding accuracy. By using ClaimLens you consent to this collection. No clinical note content or patient information is included in these signals. These signals include which codes you select, deselect, flag as incorrect, or report as missing. All data is analyzed in aggregate and never tied to individual patients or clinical encounters.
